Hospice of Greater Cleveland has formed a partnership with Eastbrook Healthcare Center to enhance services available to the communities served by both organizations. Eastbrook Healthcare Center provides skilled nursing care in Cleveland, Ohio, while Hospice of Greater Cleveland delivers hospice care and supportive services throughout Northeast Ohio.
Through this collaboration, patients at Eastbrook will have the opportunity to enter one-time contracts with Hospice of Greater Cleveland, expanding their access to specialized end-of-life care and additional support services.
According to Tiffany Moore, Hospital Administrator at Hospice of Greater Cleveland, “This partnership allows us to provide compassionate, coordinated end-of-life care that supports patients and their families, ensuring they receive comprehensive services focused on comfort, dignity and quality of life.”
