PACE acts as both the health plan and provider, covering everything from primary care and prescriptions to dental services, mental health counseling, and rehabilitation therapies. If you need transportation to appointments or help at home, it’s included. If your needs change, your care plan changes with you.
This level of coordination leads to better outcomes. Participants often experience fewer emergency room visits and hospital stays because issues are caught early and managed proactively. Even those who require nursing home–level care can continue living safely at home with the right support in place.
