Ensuring Our Residents Receive the Care They Need

McGregor’s care coordination team includes:
- Staff members from the nursing and clinical teams
- Pharmacy members
- Rehabilitation services
- Transportation
- Any other department involved in the specific care of a resident
This coordinated process is similar to piecing together a jigsaw puzzle. In its simplest form, care coordination is achieved when all parties involved in a resident’s care work together to make sure that the individual’s needs are met—during a medical visit or hospital stay, upon his or her return to one of our communities, or when at home.
Our internal care coordination team conducts multidisciplinary rounds with residents and their caregivers. These daily meetings help the resident reach recovery goals, eliminate last-minute changes in ongoing care, and prepare the resident for what is expected at home for complete recovery. We also provide a continuum of care for our residents. If at any time your needs change, you will have access to quality medical providers, specialists, and treatment options without leaving the McGregor community.
